Completing and submitting this form is the first part.

 

Submitting required documents is the other.

 

These required documents include:

  1. One copy of a cover letter giving the date of the the organization’s Board approval for the project and the amount of money being requested.
  2. One copy of your organization's most recent IRS Form 990.
